The move follows months of planning between Sharp HealthCare, the Tri-City Healthcare District, and regional regulators, culminating in a 30-year public–nonprofit partnership designed to secure the future of the region's only public hospital.
A Strategic Partnership Forged by Voters
The operational handoff became effective on July 1, 2026, following completion of all necessary regulatory approvals and execution of a comprehensive affiliation and management agreement. This landmark deal was overwhelmingly backed by local voters through Measure H, a June 2 ballot initiative that garnered 92.3% approval in the Tri-City Healthcare District. Measure H authorized the district to enter into a long-term lease and affiliation agreement with Sharp HealthCare and its wholly controlled subsidiary, Tri-City Medical Center Corporation, for the operation of Tri-City Medical Center.
Under this innovative structure, the Tri-City Healthcare District Board will retain its role as the public governing body, owning the hospital property and maintaining accountability to district taxpayers. Meanwhile, Sharp HealthCare will oversee the day-to-day operations of Sharp Tri-City Medical Center, including clinical services, staffing, and strategic planning. This "partnership model" is designed to combine Sharp’s extensive scale and expertise with vital local public oversight, mirroring a successful arrangement already in place at Sharp Grossmont Hospital in La Mesa.
Addressing Years of Financial Strain
The transition comes as a lifeline for a hospital that has faced years of financial strain and operational uncertainty. Tri-City Medical Center had previously engaged in discussions about potential closure or the downsizing of critical services, particularly since the COVID-19 pandemic, which saw the closure of its labor and delivery unit. Local officials had expressed deep concern over the potential loss of vital healthcare access for North County residents. Under the new agreement, Sharp HealthCare has committed to investing heavily in facility modernizations, seismic upgrades, and the planned restoration of labor and delivery services to restore full community care.
